Permission to Tryout forms are required for any player who was registered with another OWHA-Sanctioned team this season. This form is not needed for non-OWHA sanctioned teams, such as boys' teams.
The form can be requested any time, but is only valid after April 14th, 2026.

Please include your player's proper name and the association(s) you require a PTT for.
PTT forms WILL NOT be processed if there is an outstanding balance owing on your account or you are in possession of OGHA equipment that has not been returned.
Please allow up to 24hrs for a PTT to be issued.
